Equipment Repair Service Agreement
This agreement covers the repair and maintenance of endoscopy equipment provided by FUJIFILM Medical Systems U.S.A., Inc. (FMSU).
Covered Services
FMSU will repair and restore the equipment to its proper working condition, excluding ordinary wear and tear.
The customer must notify FMSU of any issues and may be required to ship the equipment to FMSU for repair.
Exclusions
This agreement does not cover components not manufactured by FMSU, supplies and consumables, equipment damaged by misuse or unauthorized repairs, and equipment used outside of its intended purpose.
Obligations of the Customer
The customer must provide access to the equipment, follow operating and maintenance procedures, and use the equipment as intended.
The customer must also maintain accurate records of equipment usage and provide them to FMSU upon request.
