The pump was not pre-programmed with basal rates or other verified settings, which must be set up and saved on the pump prior to use.

Risks of Not Setting Basal Rates

Failure to set basal rates can lead to severe hyperglycemia, potentially causing life-threatening diabetic ketoacidosis (DKA).

Serious injuries and one death have been reported due to not setting basal rates.

Actions Required

Healthcare professionals must assist patients in verifying and programming their basal rate settings.

Patient instructions are provided to ensure correct programming and saving of basal rate settings.
