Background
The report documents the on-site investigation of the non-deployment of all air bag systems in a 2004 Lexus GX470. The vehicle was traveling downhill when a malfunction of the braking system occurred, resulting in a loss of control and a crash into multiple large-diameter trees.
No air bags deployed during the crash, despite the presence of dual-stage frontal air bags and inflatable curtain air bags.
Investigation
The investigation included documentation and measurement of the vehicle’s exterior and interior damage, identification of occupant contact, and assessment of the manual and supplemental restraint systems.
Data was imaged from the Lexus event data recorder (EDR) using the Bosch Crash Data Retrieval (CDR) tool and software.
